It is really important that you let us know of any changes as soon as possible. This means we can offer the original appointment to another patient.

If you have any problems using this form, please call the number on your appointment letter and we will do our best to help you. You will receive a response from us so you know it has been received and actioned.

If you have recently travelled from Wuhan, China or are looking for information on the coronavirus outbreak please visit the nhs.uk website or www.gov.uk/coronavirus.
If you are experiencing symptoms that concern you, please don’t come to hospital or go to your GP practice. Please stay indoors, avoid contact with others and ring NHS 111 for advice.
Our hospitals are running as usual and you should continue to use services as normal and as appropriate.
