If you have been given an outpatient appointment at the hospitals and are unable to attend for whatever reason you can use the form below.

It is really important that you let us know of any changes as soon as possible. This means we can offer the original appointment to another patient.

Referral Guidance

Patients can be referred into the Orthotic Department by completing a GP Open Access form which can be accessed by clicking here. Please provide details regarding which orthosis is required, if this is a repeat referral, and the patient’s condition/diagnosis.Alternatively a referral letter can be sent to the department detailing the appliance(s) required.
