If you have been given an outpatient appointment at the hospitals and are unable to attend for whatever reason you can use the form below.

It is really important that you let us know of any changes as soon as possible. This means we can offer the original appointment to another patient.

If you have any problems using this form, please call the number on your appointment letter and we will do our best to help you. You will receive a response from us so you know it has been received and actioned.

Audiology

The Audiology Service is provided by a friendly and approachable multi-disciplinary team. We deliver the service across both hospital sites, Highfield Hospital (Widnes)and also in various community settings.

The Cardiac Rehabilitation service is provided by a multidisciplinary team delivering Ischaemic Heart Disease and Heart Failure rehabilitation programmes in accordance with BACPR and NICE guidelines.

The Clinical Assessment and Treatment Service (CATS) is a musculoskeletal triage service, staffed by Advanced Musculoskeletal Practitioners (AMP) who are specialists physiotherapists.

Diabetes and Endocrinology

We provide services at both Warrington and Halton hospitals and our philosophy is to support patients and health care professionals to safely manage and care effectively for patients in the community and hospital. We support patients with Diabetes and Endocrine conditions.

We provide an extensive gastroenterology service - based mainly at Warrington Hospital and providing services at Halton General Hospital as well.

Our Musculoskeletal Physiotherapy Service is provided by a team of Physiotherapists who are specialists in the treatment of muscles, joints, tendons and ligament problems.

Warrington & Halton NHS Foundation Trust provides a comprehensive multi-disciplinary Trauma and Orthopaedic service to the population of Mid Mersey, which includes fracture clinics and orthopaedic clinics, day case surgery, trauma and inpatient surgery.

The Orthotic Department provide care for those people who require Orthotic intervention or advice from skilled professionals. The Orthotic Department has vast experience in diabetic foot care, knee bracing, spinal injuries, paediatrics and vascular problems.

Outpatient Neurology Physiotherapy is a unique service which provides specialist physiotherapy intervention with a normal movement background and contemporary interventions in neurology rehabilitation.
