Cardiac Rehabilitation

The Cardiac Rehabilitation service is provided by a multidisciplinary team delivering Ischaemic Heart Disease and Heart Failure rehabilitation programmes in accordance with BACPR and NICE guidelines.

Warrington and Halton Cardiac Therapy Service (Cardiac Rehabilitation and Warrington Community Heart Failure Service) 

During the Coronavirus Escalation Period, all cardiac rehabilitation classes and face to face appointments have been cancelled for patient safety. The Warrington and Halton Cardiac Therapy Service is an integrated cardiac rehabilitation and community heart failure service. It provides a community based cardiac service which works in strong partnership with secondary care services.

Mission statement - to provide a high quality service for patients with IHD, heart failure and post heart surgery, ensuring Warrington and Halton patients have equal access to cardiac rehabilitation and heart failure support in accordance with NICE guidelines. 

Vision statement - to provide an outstanding cardiac rehabilitation and heart failure support service exceeding BACPR accreditation standards.

The Warrington and Halton Cardiac Therapy Service provides elements of the cardiac care pathways for ischaemic heart disease and heart failure which includes: 

  • A range of education services including cardiac rehabilitation from a community location.
  • A range of cardiac support services including; care planning, supported discharge for people with heart failure, disease monitoring and advice.
  • Meeting national and local guidance relating to care standards.
  • Flexible provision to meet needs at an individual level.
  • A range of delivery options which includes telephone support, face-to-face, one-to-one and group activities. 

The above service provision is delivered via:

  • Cardiac Rehabilitation Programme
  • Community Heart Failure Clinic
  • Inpatient reviews - Cardiac Rehabilitation only
  • Telephone reviews.

Clinical pathways included within Cardiac Services are;

  • acute coronary syndrome (including heart attack)
  • new diagnosis of angina
  • confirmed heart failure post - discharge follow-up
  • monitoring of unstable patients with heart failure

Referrals are accepted from primary care (cardiac rehabilitation only), secondary care and tertiary care. 

Cardiac Rehabilitation referral criteria:-

  • Adults over the age of 18 years.
  • Diagnosis with acute coronary syndrome, new diagnosis of angina, or following revascularisation/ valve surgery/ and heart transplant. 
  • Patients with stable heart failure.

Not included:-

  • Patient with an unconfirmed diagnosis
  • Stable angina
  • Syndrome X
  • Post pacemaker without heart failure or underlying ischaemic heart disease
  • Arrhythmia
  • Type 2 MI with no IHD
  • Takotsubo Cardiomyopathy without heart failure

Warrington Community Heart Failure Service referral criteria:-

  • Adults over the age of 18 years
  • Diagnosis of left ventricular systolic dysfunction of a moderate to severe rating confirmed by echocardiogram. Known to Warrington Cardiologist.
  • Other cause heart failure accepted under direct Warrington cardiology consultant supervision. 
  • Hospital admission within previous 6 months.
  • Referral to clinic via Warrington Cardiologist only.
  • NYHA II-IV

Stable patients requiring long-term heart failure follow-up and medication titration should be referred to their GP. 

Not included:-

  • Patient who pose a risk to staff as per WHHFT policies.
  • Patients with preserved left ventricular function not under the care of a Warrington cardiologist.
  • Patients with mild left ventricular systolic dysfunction. 

The Cardiac Rehabilitation Department provides the following services:

  • 8 week course for Ischemic Heart Disease patients
  • 10 week course for Heart Failure patients
  • Telephone support post discharge
  • Psychological support if required
  • Services delivered from Halton Hospital Cardiac Rehabilitation Unit and Health Services at Wolves in Warrington.

All of the above courses include exercise and health promotion sessions which cover the following topics:

  • Understanding your heart
  • Risk factors
  • Dietetics
  • Pharmacy
  • Stress management
  • Relaxation
  • Meditation
  • Physical activity
  • CPR
  • Moving on in the Community
  •  

 

The Warrington Community Heart Failure Service provides the following services:

  • Community heart failure clinics at Warrington Hospital Out-patient Department.
  • Post dischage telephone support
  • Home visit support for eligible paitents

Service provided - Warrington and Halton Cardiac Rehabilitation Service

  • 8 week exercise and education programme for patients with ischaemic heart disease
  • 10 week exercise and education programme for aptients with stable heart failure
  • Psychological support 
  • In-patient reviews for patients admitted to Warrington Hospital following a heart attack
  • Post-dischagre telephone support

Cardiac rehabilitation exercise and education sessions are delivered from Halton Hospital Cardiac Rehabilitation Unit and Health Services at Wolves in Warrington. Patients may access any site.

On completion of the cardiac rehabilitation programme, patients can be referred to community exercise programmes provided by the Health Improvement Team in Halton or LiveWire Team in Warrington.
