Stroke Service (Community) – Halton


General introduction

This community‑based service provides high‑level clinical case management, information, support and advice for people who have had a stroke, as well as their families and carers.

The aim is to improve health and wellbeing outcomes for people following a stroke or transient ischaemic attack (TIA).

How to access this service

The service is available to Halton residents who are registered with a Halton GP and have had a stroke or a transient ischaemic attack (TIA).

The service can be accessed through a referral from any health or social care professional, or directly from the patient, carer or family member, as long as the patient has given consent.

A completed community stroke referral form is preferred, but a telephone referral with the appropriate details is also acceptable.
