Neurorehabilitation Service (Community) – Halton


General introduction

The Halton Community Neurorehabilitation Service is a community‑based team of highly specialised health professionals with the expertise to provide assessment and rehabilitation for people with neurological conditions and their families.

This includes support for people with acquired brain injuries, stroke, multiple sclerosis (MS), Parkinson’s disease, and a range of other neurological conditions.

The core team is made up of neuropsychology, occupational therapy and physiotherapy.

The service works in a multidisciplinary way, with the team meeting regularly and liaising with other agencies to ensure everyone is working towards a shared therapeutic goal.

The focus of the service is to improve the quality of life for people living with long‑term neurological conditions by supporting them to achieve their optimal cognitive, physical, behavioural and emotional functioning, with the aim of maximising their re‑integration into the community.

The Halton Community Neurorehabilitation Service is based at the Independent Living Centre in Runcorn.

How to access this service

This service is available to patients aged 16 and over who have a confirmed neurological diagnosis and are registered with a Halton GP.

Referrals can be made by any health or social care professional.

Patients are offered appointments at a clinic base, in their own home, or in care or residential homes, depending on clinical need.
