District Nursing Service – Halton

General introduction

District nurses provide a 24 hour nursing service over 365 days a year providing nursing care and treatments to patients in their own homes to include residential homes.

District nurses provide care to patients who have a short term nursing need, require nursing care following a hospital discharge, have a long term condition or a terminal illness.

District nurses work in partnership with patients, carers, GP’s, hospitals, specialist nurses, community matrons, Macmillan nurses, social care teams and all other allied health professionals to help you to live as independently as possible.

Teams are based across Widnes and Runcorn in the following locations:

Widnes

Chapelfields Clinic covers:

Fir Park Medical Centre and Newtown Surgery
Beeches Medical Centre, Hough Green Health Park and Upton Rocks Surgery

Mill Brow Health Centre covers:

Peelhouse Plaza and Oaks Place Surgery
Beaconsfield Surgery and West Bank Surgery

Runcorn

St Paul’s Health Centre covers:

Tower House Practice
Grove House Practices and Heath Road Medical

Lister Road covers:

Hallwood, Brookvale Practice and Weavervale Practice
Castlefields Health Centre, Murdishaw Health Centre and Windmill Hill

How to access this service

This service is for housebound patients aged 18 or over who are registered with a Halton GP.

The service is available 24 hours a day. Referrals can be made by the patient (self‑referral) or by a GP, allied health professional, social services, hospital, hospice or voluntary agencies.

Following a referral, a district nurse will carry out an assessment to discuss your needs and agree a care plan with you.
