Adult Community Matron for Learning Disability – Halton

General introduction

We support adults with a learning disability who have long‑term or complex physical health conditions. 

Our community matrons provide specialist nursing care, help coordinate treatment, and work closely with families and other professionals to keep people well, safe and supported at home.

The service is available across the Halton area, including support in:

• people’s homes
• day centres
• respite units
• local hospitals
• care homes
• the Bridges Learning Centre
