If you have been given an outpatient appointment at the hospitals and are unable to attend for whatever reason you can use the form below.

It is really important that you let us know of any changes as soon as possible. This means we can offer the original appointment to another patient.

Please complete the form below with your contact details and as much information as possible. You can find most of the information we need on your appointment letter.

The Warrington Intensive Care Support Group was founded to allow patients and their family and friends to meet together in an informal setting and talk about their experiences and concerns. Leaving hospital and returning home is a major step in your recovery and is likely to have been a goal you’ve been working towards for some time. It is a very positive step but it will take time and effort to get back to a normal life. You won’t have the same support you had in hospital and it can be a difficult time for you and your relatives. It’s normal to go through times where you feel depressed or frustrated because you don’t seem to be getting better . Whether you’re an ex-patient or a relative, being able to talk about what you’ve been through with people who understand, because they have been through it too, can really help.
