If you have been given an outpatient appointment at the hospitals and are unable to attend for whatever reason you can use the form below.

It is really important that you let us know of any changes as soon as possible. This means we can offer the original appointment to another patient.

Please complete the form below with your contact details and as much information as possible. You can find most of the information we need on your appointment letter.

If you have any problems using this form, please call the number on your appointment letter and we will do our best to help you. You will receive a response from us so you know it has been received and actioned.

If you wish to change the date and time of your appointment you, you can click the re-book option and state any dates that are not suitable for you, or you can contact us using the number on your appointment letter.

 

Warrington Hospital’s Maternity Team welcomes all women whose care has been discontinued by One to One Midwifery.  Our award-winning Maternity Service is located at Croft Wing, Warrington Hospital and women and their babies are supported throughout their pregnancy, birth and post-natal experience by our highly-skilled team of consultants, obstetricians and midwives.
Women are supported to give birth in hospital (either in our co-located midwife-led unit, our obstetric labour ward) or at home.  At the end of the year we will open our £1m midwife-led birth centre at Croft Wing with new and improved facilities for women who choose to give birth in a midwife-led setting.
The care we provide is based on your individual needs and preferences.
We would like to assure all women booked with One to One who wish to transfer to us that they WILL be booked with our midwifery team and the best possible care provided for them and their babies.  We will turn no woman away, and have made urgent staffing arrangements to cope with the unexpected demand.
Warrington Hospital’s Maternity Service was awarded the Royal College of Midwives ‘Maternity Service of the Year’ in 2017 and the service has gone from strength to strength since then.  The recent CQC inspection gave Maternity Services a GOOD rating across all lines of enquiry.
